This article aims to outline a brief overview of Therapeutic Accompaniment (TA) in the context of Psychiatric Reform and discuss its potential as a therapeutic approach in the treatment of patients with severe mental suffering, met in public health. Specifically we will concentrate on practices developed in Psychosocial Attention Centers (PACS). The literature was reviewed in the CAPES database searching TA experiences in PACS. Although several authors suggest the TA as a promoter therapeutic practice of autonomy and psychosocial rehabilitation, aligned the care proposals within the comprehensive care for mental health users, reports of its use in the Unified Health System (UHS) are scarce. Some barriers evident for TA insertion in PACS are the lack of regulation of this practice, increased demand in services and lack of financial resources. However, we had successful experiences, as well as efforts to enter the TA in substitutive mental health services more broadly.
