In this research project, the efficiency of the surgery service of a fourth-level clinic in the city of Cali and its relationship with patient satisfaction was evaluated during the period between January 2019 and December 2022, through a output-oriented data envelopment analysis (DEA), carried out in three scenarios and using the number of employees and direct cost as input variables; and as output variables the satisfaction survey, Net Promoter Score - NPS (Net Promoter Scale), income and number of patients attended. With this study, it was possible to identify that there was a good use of resources that allowed achieving VRS and scale efficiencies greater than 90 % and CRS greater than 87 %. Moreover, it was shown that when patient satisfaction or NPS were included in the DEA as Output, VRS efficiency was excellent in all periods and higher than in the scenario in which these variables were not. In addition, the findings indicate that the NPS is a more precise output variable than the satisfaction survey since it allows identifying the factors that affect it. Additionally, it cannot be said that there was a negative impact on the efficiency of the surgery service due to the COVID-19 pandemic since the results of the scenarios differ. When the output variable of patient satisfaction was included in the DEA, there was no impact, while if this variable is excluded and only the financial, human resources and patient attended variables are left, there was an impact in some months. This study provides efficiency management strategies for health organizations both in methodology and in the selection of variables and also generates recommendations for improvement and opens the door to future lines of research with variables of satisfaction, quality and service.
